The executive producer and writer for the hit TV show Scrubs and many others has been charged with sexually assaulting five women. Investigators say there may be other victims dating back to the '90s.

Meg O'Neill, a mentor to young refugees, died after pushing a young woman into safety. They were ascending the frozen Raven Falls in Utah with another climber when the ice cracked, police said.
